Project SCOPE
Through individualized care planning and social services, Project SCOPE (Senior Community Outreach Program to the Elderly) strives to enable older people to live as actively and independently as possible in their own homes. After a comprehensive assessment of the older adult’s needs, SCOPE workers coordinate a range of services which may include advocacy, transportation, counseling, friendly visiting by volunteers, financial management assistance and home care referrals for those who qualify.

Services are provided for individuals who live between 14th and 96th Streets, from Fifth Avenue to the East River, and on Roosevelt Island.
